GENERAL SUMMARY : The Senior Post-Acute Care Coordinator is responsible for driving admissions for CRISTA Rehab & Skilled Care meeting census goals and objectives identified by leadership. Perform pre-admission clinical evaluations to ensure referral goals are met. Capture and admit appropriate patients in a timely manner. Work closely with the Administrator and Director of Marketing as the first line of assessment for post-acute and skilled patients within the Puget Sound area. Function as the sales advocate while giving fair computation for discharges within referring hospitals.

ESSENTIAL JOB DUTIES:

Admissions

Clinically assess each hospital discharge to determine the appropriateness for admission. Apply sound judgement and knowledge of post-acute admission department standards. Review medical documentation as approved, collaborate with case managers, discharge planners and coordinate admissions with the nursing and admissions department. Inform admissions of any special requirements for admission during discharge. Comply with the number of admission required as monthly goal by supervisor. Use discretion and independent judgment as it relates to the admission department in a timely and efficient manner.

 

Admission Referrals

Act as initial point of contact for hospitals; build and fosters relationships. Manage primary referral hospitals and ensures a smooth discharge process for patients transitioning from a hospital setting to our center. Market to healthcare providers who may benefit from CRISTA’s services. These contacts include nurses, physicians, case managers and social workers. Meet referral/admission goals set by Administrator and Marketing Director. Develop strong relationships with key referral hospital personnel and identify new opportunities for growth.

Work collaboratively with supervisor, coworkers and customers.

Perform other related duties as assigned.

ESSENTIAL QUALIFICATIONS:

Christian Commitment: A deep and abiding faith that is fully surrendered to the will of Jesus Christ, evidenced by a lifestyle that is consistent with biblical principles in word and deed. Commitment to spiritual growth and development. Demonstrates the values of a CRISTA leader.

Education: Bachelor’s degree in Physical/Occupation Therapy, Marketing, Sales or Bachelor’s of Science in Health Sciences or a degree in Nursing or a related field from an accredited college or university recognized by the United States Department of Education or equivalent.

Experience: two years fulltime or equivalent of hospital clinical work.

Five years’ experience in healthcare sales and marketing.

 

Licensure/Certification:

Registered Nurse or EMT Certification or equivalent healthcare certifications.

 

Software and Equipment Used:

Point Click Care, Epic, MS Office Suite, Genesis or McKesson.

  • Strong leadership skills, self-confident and able to take control of a situation whenever necessary.
  • Able to perform well under pressure while maintaining calm.
  • Strong knowledge and understanding of the post-acute clinical model.
  • Sound and accurate judgement.
  • Innovative and self-driven.
  • Strong planning and organizational skills, effective at getting things done.
  • Collaborative, able to involve others as appropriate in decision-making processes.
  • Able to adapt in a changing environment.
  • Effective at finding solutions and completing tasks and projects on a timely basis.
